A large Healthcare Company was experiencing significant quality and reliability issues with critical applications. This was due to best practices violations including performing code deployments during the regular business hours. There was limited documentation surrounding the application policies, procedures, maintenance, and service level management. In addition, problems and incidents were not tracked in a consistent manner and there was a lack of infrastructure redundancy/failover, creating many single points of failure.
